体外胚胎的临床评价从原核观察开始，主要观察内容包括原核和核仁的特征，主要观察原核的数量、大小和相互位置，以及核仁的数量、排列和对称性，原核的形态、核仁的大小和排列、细胞质的出现等形态特征与胚胎发育、着床率和妊娠率有关，原核观察期为精卵共孵育后16 ~ 18小时，双原核的出现提示受精正常，原核的发育速度和对称性与胚胎的发育潜能有关。
什么是胚胎原核
胚胎原核也指原核细胞的核质体，是精子进入卵子后的配子，排卵后，卵子进入输卵管壶腹部，当获能精子与卵子的透明带结合时，受精就开始了，受精在输卵管壶腹部需要24小时左右，此时的受精卵称为原核胚胎。
当精子进入卵母细胞时，精子形成雄性原核，卵子形成雌性原核，两个原核携带来自双亲的遗传物质，逐渐靠拢形成两个原核的合子，也叫受精卵，这个阶段的受精卵自然会在输卵管内逐渐融合，然后进行有丝分裂和增殖，形成卵裂期和囊胚。
Tips：原核胚是指体外受精后16 ~ 18小时观察到受精卵有两个原核胚，对于一些重复胚胎评分低，没有胚胎可移植的患者，可以尝试原核胚胎移植，但需要注意的是，原核胚胎移植并不是一种通用策略，并不适用于每一位患者，而是针对反复无胚胎可移植的患者的一种试验策略。
