胎儿系统超声筛查是畸形检查的一种，常被称为四维或三维彩色多普勒超声。3D看到的是图片，4D看到的是视频，都可以排除胎儿畸形。该测试可以在孕中期或孕晚期进行。孕中期做胎儿系统超声筛查的时间在22周到26周之间，孕晚期做这个检查的时间在28周到32周之间。这时候主要检查宝宝是否有心脏异常。胎儿超声筛查还可以检测出孕妇和胎儿的一些疾病，如心脏瓣膜病、先天性心脏病、宫内血栓形成等。
4D比3D和2D更好吗？
这是个误区！“三维比二维多一维，肯定比二维好；四维比三维多一，肯定比三维好。”三者中，二维超声分辨率更高，对胎儿结构畸形的判断能力高于三维和四维，所以，二维是他们的基础。
- 在怀孕中期，用过筛查来判定宝宝的形态时，三者的应用都是相互影响、相互帮助的。当遇到某些胎儿体表异常时，三维、四维超声可能会提供一定的帮助，辅助超声医生进行诊断，让孕妇及其家人更直观的了解胎儿的异常情况。
此时，3D超声成像往往比4D多使用，因为3D图像作为静态图像，往往显示出比动态图像更清晰的结构和病变。
- 四维超声常用于动态观察某一结构的运动功能和变化。所以不能说“三维”比“二维”好，“四维”比“三维”好。一般来说，二维超声是宝宝畸形排查的首要方式，而三维彩超、四维彩超则是补充的部分，它们之间相辅相成。
为什么要在特定时间做排畸筛查？
宝宝的畸形筛查要在特定的时间去，一般是在20周到24周之间是最好的。因为这个时候宝宝的大部分器官已经生长完善了，是检查的最佳时间。这个检查可以看到宝宝全体的状态和生长情况，是上到下、从内到外的一个系统排查。
从中可以发现宝宝头部、内部的心脏、肺等部分的情况，如果孕周太小不便于医生观察、有肢体方面的畸形很可能会疏忽，但是孕周太大的话如果发现严重的畸形孕妇就会面对较大的风险来引产。20-24周做大排畸检查可以排除60%-70%左右的结构畸形。
